Annual Children & Youth Permission Form 2025-26

By completing and submitting this form, I (parent/guardian) give permission for my child/children (from birth up to 18 years) listed below to participate in the activities of Faith Bible Church including but not limited to Sunday morning activities, Wednesday evening activities, and all other children’s, youth, family or women's sponsored events or activities. At Faith Bible Church we require new Children & Youth Permission Form to be submitted each school year. This permission is effective from the date signed through August 31st, 2026.

This Release and Arbitration Agreement is made by and between Faith Bible Church and the undersigned parent/guardian of the below named child or children in consideration of Faith Bible Church providing child/youth ministries, activities, and programs organized by Faith Bible Church (FBC) to the undersigned or the undersigned’s child or children. In case of an emergency in which my child/children need immediate medical attention or surgery, and after reasonable efforts have been made to contact me or my spouse, and we cannot be located; I hereby give permission to FBC, a duly licensed medical provider, EMT, Nurse, Physician or other licensed medical professional selected by an agent, representative, or adult leader of FBC in charge of my child, to secure or approve proper medical treatment of any kind, including hospitalization, anesthesia, surgery, or injections of medication to my child. The undersigned forever waives, releases and discharges FBC and its pastors, agents, employees, and volunteers from any/all liabilities, injuries (including death), claims, disputes, or actions resulting from my child’s involvement in the child/youth activities and programs provided by FBC for the benefit of my child, regardless of the location from the date of execution of this document until the termination date identified below. I/We hereby agree to indemnify and hold harmless FBC, its pastors, agents, employees, and volunteers from and against any and all claims, liabilities, and obligations of every kind, nature, and description, contingent or otherwise arising from or related to the participation of my child/children in the child/youth programs provided by FBC. I attest and verify that I have full knowledge of the risks and dangers involved in my child’s/children’s participation in the child/youth activities and programs provided by FBC; that I assume such risks, and that I will assume and pay my own medical and emergency expenses, in the event of an accident, illness or other incapacities, regardless of whether I have authorized such expenses. Any controversy arising out of, connected to, or relating to the care of my child by FBC or participation of my child in any child/youth activity or program of FBC, this Release/Arbitration Agreement, or the breach thereof, including, but not limited to any claims of violation of Federal and/or State Law, as well as any common-law claims, shall be settled by arbitration. In accordance with this paragraph, a judgment based upon the arbitrator’s award may be entered in any court having jurisdiction thereof in accordance with the provisions of R.C.W. 7.04. This agreement shall be construed and interpreted under the laws of the State of Washington. BY COMPLETING AND SUBMITTING THIS FORM, I AM STATING THAT I AM A PARENT/GUARDIAN OF THE ABOVE-LISTED CHILDREN/YOUTH AND HAVE FULL PARENTAL AND/OR LEGAL AUTHORITY TO EXECUTE THIS RELEASE INDIVIDUALLY AND WITHOUT THE CONSENT OF ANY OTHER PERSON OR ORDER OF ANY COURT. I HAVE READ THIS WAVIER AND RELEASE CAREFULLY AND UNDERSTAND AND GIVE PERMISSION FOR MY CHILD/YOUTH TO PARTICIPATE IN THE ACTIVITIES ASSOCIATED WITH THE CHILDREN'S/YOUTH MINISTRY PROGRAMS OR ACTIVITIES OF FBC FROM THE TIME THIS FORM IS SUBMITTED UNTIL AUGUST 31, 2026.
